All new client forms will be made available through an online portal link that I send you via email after your initial free phone consultation and before your first appointment.
You will be invited to complete the intake forms online and sign them with an electronic signature. All forms are required to be completed prior to your first appointment.
We are saving time and trees by having no paper forms to manage. Your data is always protected with HIPAA-compliant software and security.

For Current Clients
For established clients, you should have already received a secure link to my online portal. You will receive reminders 48 and 24 hours prior to appointments. You will also receive the portal link 15 minutes before telehealth appointment start times. A new telehealth link will be send for each separate appointment. If you have questions about accessing the portal or signed forms, please call or email your therapist.
